December 12, 1969 - June 22, 2020
Jeffrey Roger Clark, 50, passed away on June 22, 2020. He is lovingly remembered by his wife, Lesley Baxendale Clark and son, Nicholas Clark (14) of Thonotosassa, FL along with his parents, Roger and Sandra Clark of State College, PA and his sister, Jennifer & Scott Pickering of Emmaus, PA. Jeff was also dearly loved by Lesley’s father, Jim & Annette Baxendale and close sister-in-laws Laura “Sissy” Maranto, Donnette Figueroa and Jackie Baxendale all from the Tampa area. Born in Oneonta, NY, Jeff spent his childhood in the northern mountains and among numerous friends in Happy Valley. He was a 1988 class grad from State College Area High School and went on to receive a Bachelor of Science Degree (1988-1992) and a Master’s Degree in Audiology from Penn State University (1992-1995). Jeff headed south and began practicing Audiology in Hillsborough County. Dr. Clark was clinically certified in Audiology by the American Speech-Language Hearing Association. He was a fellow of the American Academy of Audiology and the Florida Academy of Audiology. Jeff received his Doctorate degree at the University of Florida in 2000 and continued to work with several well known ENT specialists throughout Tampa. He also evaluated and treated many patients who suffered with dizziness and balance disorders. In order to bring a better quality of life to thousands of patients in the area, Jeff started his own practice Physician’s Choice Hearing & Dizziness Center, Inc. in 2002 with dear friends Dr. Scott Sims and Lesley Clark. He also spent a great deal of time mentoring future Audiologists through the University of South Florida Academy and internship program. Jeff was extremely thankful for his office family and friends who helped the practice thrive, Olga Cortes, Kara Fagan and Jen “JJ” Josi. Dad, I will never forget the wonderful times we spent together or all of our” adventures”. We shared a love for the outdoors; biking, archery, shooting, camping, fishing and traveling. I’m thankful for the trip we took last year to Gatlinburg, TN and stayed in that amazing cabin in the woods. I enjoyed us playing guitar together, jokes and the occasional pranks on mom. You taught me so many things and to always tell the truth no matter what. I will always remember you as brave and kind. I love and miss you. Jeff, I will always remember you as being kind, generous, compassionate, respectful, devoted, loving and caring. I don’t think that you could have ever imagined the impact your absence would have on our lives. There was still so much left for you to do but I know God had a different plan. Although you aren’t here with us now, you continue to live on in our hearts. I know that you are shining down on Nick and I and will continue to watch over us from heaven. I will miss you forever. Spending his entire career in the healthcare field, “Dr. Jeff” was genuinely grateful for his compassionate caregivers at Advent Health Hospital ICU, Home Instead and Hospice. Thank you to Alexis, Lacey, Bri, Rachel, Zee, Alyssa, Kesha, Jessica, Jenn, Faith, Kelli, Marie, Jenna, Mary, Barnett, Kash, Patricia, Linda and Jill. There will be a private memorial to celebrate Jeff’s short, but meaningful life among those who loved and laughed with him. Since we are unable to have a large crowd gathering, Jeff’s Celebration of Life will be held at a later date.
